How to obtain copies of your medical records?

While you have the right to access your information, the medical chart is the property of Spectrum Orthopaedics.  If you were referred to us by your primary care provider, a copy of your office visit notes will be automatically forwarded to him/her.

If you desire a copy of office visit documentation or images to be sent to yourself or another entity, you must complete an Authorization to Release Protected Health Information form.  A completed form (inclusive of signature and date) is required for ALL such requests.   Forms are available below to download and print.

PLEASE NOTE: if you had an MRI or imaging done at CDI/Rayus Radiology at 33 Sewall St., you can obtain your records by clicking here.

How long will it take for my request to be processed?

Processing times may vary depending on the complexity of the request; however, in most cases requests are completed seven to ten business days from the date of receipt. Please note that per HIPAA guidelines, facilities have 30 days to complete requests. If you need to speak with someone regarding a request you have submitted, please allow 24-48 hours before calling.

Is there a charge associated with my request for copies of records?

In accordance with State of Maine Statutes, there may be a “reasonable cost-based fee.”   If it is deemed that any processing fees apply, we will contact you before completing your request.

If you have an upcoming appointment with another physician or medical provider, someone from his/her office may contact us directly as laws allow us to release records to them without the need for you to complete a form.
